To provide you with a Sozo session we must ask you to read and agree with our "liability release" terms shown below. Please read them. Once you have read them you must tick the box to indicate your agreement.
I release Bethel Sozo UK and Kingdom Life church, Norwich and its ministry volunteers from any liability for any harm resulting from my voluntarily receiving of ministry on this and subsequent visits. I understand that the ministry of Bethel Sozo UK is staffed by volunteers and that they are not trained or licenced professionals of counselling, therapy or medical services.
I undertake that if I am currently taking medication, or operating under the advice of a professional service, I will consult with my medical doctor, therapist, counsellor etc. before altering any prescribed course of medication or action.
I further state that I have voluntarily sought assistance at my own initiative and that I am under no obligation to accept or reject any of the help that I might receive from the team members during my appointment. I understand that these team members are, to the best of their ability, doing what they can to help me achieve more freedom in my life.
I also agree to hold Bethel Sozo UK and Kingdom Life Church, Norwich free from any and all liability, loss or damage of any kind that may arise as a result of assistance that I have received or from my involvement with them
Confidentiality
I understand that my sozo session will be treated as confidential.
However, in certain circumstances the sozo team members are legally bound to pass on information to the relevant authorities if a person is a risk, or certain criminal acts are disclosed. For instance, the Children Act 2004 requires any confidential disclosures in respect of current abuse of a child or vulnerable adult to be reported to the Social Services Department. This will be done in accordance with the safeguarding protocols of Kingdom Life Church, Norwich.
During ministry appointments, team members may take notes to aid them and/or to be given to you at the end of the session. Following the session, the team member will destroy their copy of any notes. The only record of your sozo session will consist of the good news truths that you heard from Heaven. These will be given to you at the end of the session or soon after electronically and then any copies will be destroyed once you acknowledge receipt.
If the sozo session is conducted online using apps such as Zoom or Skype, we undertake not to record the sozo session. In submitting this form you also agree that you will not record the sozo session.
Before & after your sozo session
Once we receive your form we will be in touch within a week to agree a time & date for your sozo session.
It can be helpful to fast or pray in the days leading up to your Sozo.
Forgiveness is foundational to your sozo session. If you choose to harbour unforgiveness then we will not get very far in your sozo. Please read our sheet on forgiveness HERE
We recommend that you share with someone you trust what happened during your Sozo so that you will have someone to pray with and hold you accountable. This person should be a “safe place” but not necessarily someone you consider your “best friend”.
